SIAI Dislocation

SIAI dislocation (in children):in textbooks  this lesion should be treated conservatively and not by surgical reduction. Is this correct?

Conservatively healed dislocations always show a prominent callus. If it is true that extraarticular impingement of the hip will cause degenerative changes, we should treat the SIAI dislocations surgically: reduce them and fix the spina, avoinding healing by prominent callus. When we operate older patients who had in the past a SIAI dislocation, we must admit that they reached an advanced age before prosthetic replacement was necessary, but they always referred reduced flexion on the injured hip. The hip which needed prosthetic replacement was always the injured one and not the controlateral.

On the other side: surgical fixation is easy, small incision (in the Hueter intervall) and low morbidity.
